In May 4, 1969, an M113 armored personnel carrier (APC) from F Troop, 17th Cavalry, 196th Infantry Brigade was on a combat operation when it hit a mine 2 ½ miles west of Que Son, Quang Nam Province, RVN. The blast and subsequent fire killed four soldiers traveling in the vehicle including commander SP4 Joseph R. Karpy, gunner PFC Richard D. Sweet, crewman PFC Gregory A.
